Your role

This is a very special role to provide holistic health and wellbeing of those who visit our open drop-in at the Vineyard Community Centre in Richmond. To us, they are our guests, not 'clients', 'patients' or users'.

This will include personal health screening, and holistic health support; also to provide health education, and liaison with outside agencies and local surgery. Many of our guests have multiple and complex needs involving, mental health, drug/alcohol dependency and homelessness.

You’ll report to our Community Nursing Manager and work with our Case Worker and Caseworkers from the Glass Door charity, and external hosted partner organisations like SPEAR and C.A.B.

Age UK East London are recruiting a Community Engagement Officer to join our growing Community Outreach team. We support hundreds of people a year to maintain their health and wellbeing, combat social isolation and digital inclusion through the varied activities and by signposting to relative local support services.

Job description 

Job Purpose 

• To provide a high-quality outreach support service that enables older adults with low level support needs to live independently within the community. 

• To a promote network services across the Boroughs to maximise their take-up. 

• To work closely with statutory and voluntary sector agencies to ensure appropriate services are accessed. 

• To ensure that service users have maximum choice and control over the planning and management of the service they receive, including initiating individual support plans and connecting them with culturally appropriate services. 

Key Tasks 

• To signpost people to appropriate services as required through outreach sessions in the Hub, Satellite centres, GP practices and pharmacies. 

• To develop shared working practices across the service network. 

• To work closely with providers of similar services 

• To work to agreed individual, team, and organisational objectives. 

• To work with existing activity groups at the hub and satellite centres and support the setting up of activities where appropriate. 

• To ensure all contacts with service users are recorded appropriately and to maintain an adequate filing system. 

• To use word-processing software to produce own correspondence and reports in accordance with relevant policy on confidentiality and data protection. 

• To collect monitoring information to enable the service to be evaluated. 

• To provide support to services outside of normal working hours and occasionally attend sessions when required. 

Administration  

• To ensure all relevant administration is organised and efficiently completed. 

• To assist co-ordinating service user client surveys/questionnaires as appropriate, collate feedback and evaluate the service provided. 

• To assist with the collection of data necessary to provide specific information needed for KPI’s and external requirements and outcomes. 

• To ensure all data is collected through AUKEL organisational customer relationship management data base systems. 

Quality 

• To be familiar with and to implement Age UK East London’s policies and procedures. 

• Ensure that confidential data is managed within AGE UK’s policies and procedures.

 General 

• To meet regularly with line manager for support, supervision, and appraisal. 

• To attend team and staff meetings, (and other meetings) as required.

The Role

The Community Support Worker will be required to work alongside the full-time area Co-ordinators to support activities and groups and carry out one-to-one initial appointments for people residing in Surrey.

This will involve attending some groups in venues across the Surrey Downs area, to assist in the facilitation of groups to ensure they run safely and meet the needs of the clients.

The role will also involve supporting the full-time co-ordinators to complete initial appointments with new clients, including the completion of recovery stars and plans and updating client records.

About the Organisation

The charity is a mental health and emotional wellbeing charity, supporting Surrey residents since 1994 in Banstead, Elmbridge, Epsom & Ewell and Mole Valley.

Offering a variety of face-to-face and online services, available to adults (aged 16+) living in Surrey who would like to restore and/or maintain their mental and emotional wellbeing (no diagnosis needed). The services are free of charge, with no waiting list and no time limit.

Based in our Welcome Area, this role is central to our Community Support provision, where the post-holder will be our community's first point of contact, responsible for greeting service users and supporting them with general enquiries and referrals.

Supported by a team of volunteers, they will manage community enquiries, ensuring IRMO is always a welcoming environment for the community. Summary of Role

This is an exciting new role in our Talking Therapies team providing vital support to people in Manchester with common mental health problems. Working alongside counsellors, you will provide wrap-around social and practical support to adults on our waiting list for or who are engaging in Talking Therapy to enable them to address and overcome barriers impacting on their health and wellbeing. You will provide advice, information and guidance on these issues, make onward referrals to specialist advice organisations where necessary and support engagement with these services. 

You will build and maintain relationships with key referrers, such as GP surgeries, to promote the service and facilitate appropriate referrals.

Once trained, you have the exciting task of leading and facilitating a new course within the service: Living Life to the Full™, a CBT-informed group programme designed to support people to improve low mood, overcome stress, sleep better and build confidence.

The Hepatitis C Trust develops projects nationally where peers use their lived experience of injecting drug use and hepatitis C, to provide education and training, increase hepatitis C awareness, and access to testing and treatment for people living with hepatitis C. 

Working with the South West Peninsula ODN, The Hepatitis C Trust are now expanding this commitment through the introduction of the HCC (Hepatocellular Carcinoma) surveillance and early liver disease detection programme.

We are looking for a self-motivated individual who can work on their own, engage with stakeholders and have a desire to make a difference in promoting liver screening in community settings, and by increasing access to specialist care services.

Working in partnership with local hospital and community outreach clinics, the post holder will ensure that patients are supported through initial liver assessment and ongoing surveillance as required. The post will coordinate delivery of educational and awareness workshops and promote the delivery of liver screening in community settings 

Working under the guidance of the South West Peninsula Peer Programme Manager, the post-holder will work alongside other identified partners to help develop a community outreach liver surveillance program and carry out HCC early detection activity and interventions across Plymouth.

We require a good standard of education, willingness to travel and welcome creativity and innovation in all our work. We are a patient-led organisation – you will be working in an environment where the patient/service user/client is placed at the centre of all that you do. This post will require extensive travel across the region and the post holder is required to hold a clean driving licence and have their own vehicle for work related travel.

We are River Action

We are a small and agile campaigning organisation on a mission to rescue Britain’s rivers from a toxic cocktail of agricultural, sewage and industrial pollution. We support community action, galvanise and mobilise public opinion, influence government policy, and call out destructive industrial practices to incentivise changes that end pollution. 

We are building a talented team of positive disrupters and community mobilisers to deliver our strategy to urgently protect and restore our rivers for the benefit of community and wildlife.

Who we are looking for:

We are looking for highly engaged people who share our passion for rivers, wildlife and people who bring a creative, collaborative and investigative approach to campaigning. For this role we are looking for a confident and resilient community organiser who can build strong relationships to support communities to create change locally.

What you’ll be doing:

The Communities Coordinator will support community groups to campaign for clean rivers at a local level. By facilitating grass roots action you’ll be galvanising a strong and powerful network of community activists that have the power to create change both locally and nationally. You’ll be responsible for taking River Action’s national campaign strategy to a local level. You’ll work closely with the Campaigns Manager and the wider team to develop a Community Toolkit (including advice for activists, and guidance and services for communications, fundraising, legal support and citizen science projects) to support communities as well as provide strategic support to empower community activists to campaign for clean rivers. You’ll work closely with the Campaigns Manager and wider team to amplify the stories of those taking action at a local level and connect campaign groups to the wider national agenda. You’ll be working collaboratively with the River Action team to devise, develop and deliver our strategy ensuring our programme of community engagement is equitable, diverse and inclusive. You’ll be working closely with other organisations to ensure our community support compliments the work of others and meets the needs of community groups we work with. 

Main Duties 

• Contribute to the development and implementation of River Action’s campaign strategy.

• Develop River Action’s Community Toolkit as a package of support for local campaign groups. 

• Identify community groups within river catchments areas River Action is working and build positive and impactful relationships..

• Organise site visits to develop good relationships and in-depth understanding of local issues impacting river health.

• Convene and align local stakeholders around river catchment and regional campaigns.

• Facilitate local community groups’ campaigns and provide support in line with our toolkit’s support package.

• Facilitate the creation of community group citizen science projects, integrated within a national system for gathering and sharing evidence of pollution.

• Organise community crowdfunding for citizen science and communications.

• Plan and host webinars, training sessions and online and in person events.

• Develop good working relationships with regulators including Environment Agency, Natural Resources Wales and Scottish Environment and Protection Agency. 

• Support the CEO and Campaigns Manager to identify and deliver legal challenges at a local level.

• Maintain and develop partnerships and working relationships with other key groups and stakeholders working in similar areas of work to River Action.

• Identify opportunities to engage, empower and expand River Action’s network of supporters. 

• Support the Communications Coordinator to create inspiring and engaging digital media content and news stories, including taking photographs and videos and conducting interviews.

• Support the Campaigns Manager and Communication Coordinator in responding to the reactive news agenda.

• Contribute to the Monitoring and Evaluation system for demonstrating impact of River Action’s community engagement work, reporting against KPIs. 

• Contribute to fundraising efforts particularly crowdfunding and drafting of funding proposals to support Campaigns and Fundraising Teams and the charity’s mission.

• Work as a motivated and collaborative team player within a fast-paced team.

• Carry out any other duties as are within the scope, spirit and purpose of the post as requested by your line manager or any other member of the River Action team.

Other Duties & Responsibilities 

• Adopt a positive approach to personal and professional development; engaging in one to one meetings and relevant training.

• Be aware of, and act on, relevant health and safety responsibilities as an employee of River Action and adhere to these wherever you are working.

• To maintain confidentiality in all areas of work at River Action.

• UK travel will be required. Training & operational activities may require overnight stays away from home, in which case standard River Action procedures apply.

• Work collaboratively with colleagues across River Action to collectively deliver our ethos and mission.

• The ability to communicate effectively and appropriately with a wide range of people both verbally and in writing including the media, community groups, other NGO representatives, supporters and followers on social media.

• Ability to travel to remote locations not necessarily well-served by public transport.

This year, River Action will be developing its Communities Toolkit aimed at local campaign groups and activists seeking to develop their own local campaigns to rescue their rivers locally. As part of your application, we would like you to put together a one page document outlining a plan for the toolkit, mapping out the main structure and content of the toolkit and how you would deliver it to communities. We will particularly be looking at your style of communication and creative skills. Should you be successful in being invited to interview, you may be asked to present this idea.
